Bring a large pot of salted water to a boil. Wash and dry produce. Halve, peel, and thinly slice onion.
Heat a drizzle of oil in a large pan over medium-high heat. Add onion and season with salt and pepper. Cook, stirring, until softened and lightly browned, 5-6 minutes. Add 2 tsp sugar and ⅓ cup water (4 tsp sugar and ½ cup water for 4 servings); cook, stirring, until onion is caramelized and jammy, 3-4 minutes. Taste and season with salt.
Once water is boiling, add pasta to pot. Cook, stirring occasionally, until al dente, 9-11 minutes. Reserve 1 cup pasta cooking water, then drain.
While pasta cooks, add sausage to pan with onion. Cook, breaking up meat into pieces, until sausage is browned and cooked through, 4-6 minutes. Add tomato paste; cook, stirring, until combined, 1 minute. Stir in ½ cup reserved pasta cooking water. (Ladle water directly from pot if pasta is still cooking). Stir in cream cheese until melted and combined, 30-60 seconds. Reduce heat to low. Season with salt and pepper.
Add drained pasta and 1 TBSP butter to pan. Toss until butter has melted and everything is thoroughly coated in sauce, 1-2 minutes. (If sauce is too thick, add splashes of reserved pasta cooking water as needed.) Season with salt and pepper. Divide pasta between bowls and top with Parmesan.
